Who needs IVF?

A woman’s egg quality diminishes with age. IVF can give an older woman a better chance to conceive by increasing the number of eggs she produces, enhancing selection of good quality eggs, or by allowing her to use donor eggs to create embryos.
Depending upon the underlying issue for recurrent pregnancy loss, IVF may be an effective treatment in helping to begin a successful pregnancy by increasing the number of candidate embryos and possibly using PGD or PGS to enhance successful implantation.
One fifth of infertile couples are diagnosed with unexplained infertility, and these couples often achieve pregnancy using IVF.
If a woman is not ovulating (anovulation), ovulation-inducing pills often aid in achieving pregnancy. But if she is unable to achieve pregnancy with these conservative measures, IVF can be used, which has very high success rates.
If significant tubal damage exists, the only fertility treatment options are surgical repair, which has a low success rate, and bypassing the fallopian tubes using IVF.

Endometriosis can cause infertility but can be successfully treated using a combination of surgery and medication. If the first round of surgical or medical treatment fails, IVF is an appropriate next step.
If a woman has a low number of eggs in her ovaries, the IVF process can be used to stimulate healthy eggs to develop, allowing the reproductive endocrinologist to collect mature eggs directly from the ovaries. In particular, younger women with low ovarian reserve who generally have good egg quality would have better success rates with IVF treatment.

The screening includes some medical tests like semen analysis, ovarian reserve testing, check on your medical history if any infectious disease like HIV, AIDS. Ovarian Stimulation
 

IVF cycle begins from this stage which involves several junctures like ovarian stimulation, egg retrieval, sperm retrieval, inseminations and embryo transfer. One IVF cycle may take 2-3 weeks approximately. Female partner is provided with the medications to stimulate ovaries for egg growth. Only some of them will get enough mature to get fertilized. Few medications may be used as to put a stop to pre-mature ovulation, for preparing the lining of your uterus for that you may recommended by your doctor to begin taking progesterone. Some injections might be given to you containing FSH (Follicle-stimulating hormone) or LH (luteinizing hormone) or combine to stimulate egg quantity.

Growth Monitoring
A visit to your doctor for the ultrasound monitoring of egg size, at this stage the size of the egg is to be monitored by vaginal ultrasound to ascertain the growth of egg. Moreover, blood tests will be conducted to evaluate the response to your ovary because of the medication, usually the progesterone levels remain low whereas the estrogen level increase with the development of follicles. Afterwards, a final trigger shot is given for the maturation of eggs once the egg reaches 18-20 mm size.
Egg Retrieval
later eggs are collected from the female’s ovary at clinic 1-2 days after the final trigger shot. This process is painful but anesthesia will be given to avoid discomfort. A thin needle is passed from the vagina under ultrasound probe to get the ovary and follicles. With the help of suction device, the eggs detached from the follicles wall are to be sucked out of the ovary. This usually takes 10-15 minutes. This process may make you feel mild cramping just like your experience in menses. From the collected egg, mature and healthy eggs will be placed with the sperm in order to get them mix and resulting in embryo.
 

Sperm Retrieval
 

Either donor’s sperm or your partner’s sperm can be used as per the situation takes you. If it’s your partner then, your partner has to provide the semen sample at the doctor’s clinic by masturbation or in either case, it can be extracted directly through the testicles. Same is the process for the collecting the donor sperm. Once the sperm is collected, it is separated from the semen fluid at the laboratory.

 

Fertilization
 

The insemination can be carried out by various methods as per requirement. In conventional fertilization, the healthy eggs and healthy sperms are kept overnight under the controlled lab environment to form embryo. Whereas, in Intracytoplasmic Sperm Injection, a healthy sperm is directly injected into the mature egg. They are kept under observation until it results into a healthy embryo. If the embryo is not healthy then it may lead to failure of IVF cycle.

 

Embryo Transfer
 

After 3 to 5 days of Fertilization, embryo(s) is/are implanted to female’s uterus where it attaches itself to the uterine lining for pregnancy to occur. It is transferred by long thin tube on which embryos are attached at the end of catheter and inserted through your cervix into your uterus. You may feel mild bloating, moderate cramping, constipation. If the pain is extreme or severe, then you immediately contact the doctor to overcome the complications.

 

Pregnancy Test
 

After 2 weeks of embryo transfer, blood test is done to check the implanted embryos. There can be two possibilities: first, if you are pregnant, your doctor will guide you about prenatal care. Another if you are not pregnant, then you will get your menses within a week or you may bleed for some days in either case. If you don’t get your menses then contact your doctor in order to identify your complications.

In vitro fertilization (IVF) is the most common type of assisted reproductive technology (ART), used to create an embryo by bypassing certain causes of infertility, such as mild sperm abnormalities in men, and fallopian tube or ovulation irregularity in women. During IVF, a woman’s eggs are surgically retrieved and then fertilized in a laboratory by mixing with a partner (or donor’s) sperm. The fertilized egg, or embryo, is left to grow for two to five days, and is then surgically transferred back into the woman’s womb.

In general, patients will seek advice from a fertility doctor after one year of trying unsuccessfully to get pregnant. The chances of a fertile couple conceiving a child in any given month (called the natural pregnancy rate) is around 20%; resulting in roughly ninety percent of couples becoming pregnant after one year of trying to conceive. It’s recommended that the remaining 10 percent of couples consult a fertility specialist. In particular, women over thirty are encouraged to undergo a fertility treatment evaluation, after six months of attempting to conceive. And it may be beneficial for women over forty to meet with a fertility doctor shortly after deciding to try and have a child.

There are a number of factors involved in determining if a patient is suitable for IVF treatment. Appropriate candidates often include couples who may experience:

a) Low sperm counts
b) Endometriosis
c) Problems with the uterus or fallopian tubes
d) Ovulation disorders
e) Sperm unable to penetrate or survive in the cervical mucus
f) Other health or unexplained reproductive issues

The only way to know for sure if in vitro fertilization is right for you is to undergo a complete exam and consultation.

The in vitro fertilization process can last anywhere from four to six weeks prior to egg retrieval. The embryo(s) will then be implanted between two to five days afterwards. Not all patients are successful on their first IVF attempt; in fact, it’s not uncommon for patients to go through multiple IVF cycles before finally becoming pregnant.
